							<li>Take a Printout of the Completed Form.</li>
							<li>Affix your passport size Photo.</li>
							<li>Sign the Form.</li>
							<li>Get the signature of an existing member of Maheshwari Pragati Mandal, as a Proposer</li>
							<li>Attach the following documents to the Application Form:<br>1. Aadhaar Card<br>2. Cheque for Rs 1180/- (including GST) drawn in favour of Maheshwari Pragati Mandal, Mumbai</li>
